Spontaneous uterine rupture: a case study.

Spontaneous rupture of the gravid uterus is a serious and life-threatening complication of pregnancy for both mother and baby. With the increase in cesarean births over the past decade, this complication will be seen more frequently. Spaulding and Gallup reported 15 cases of spontaneous rupture of the gravid uterus over a 21-year-period (or an incidence of 1:6673). Of these patients, 47% had previous cesarean sections, and 13% had received oxytocin (Pitocin®) prior to rupture. Fetal mortality occurred in 13% of the cases, however, no maternal deaths resulted. They also reported that 40% of these patients were able to undergo uterine repair, while the remaining had hysterectomies. In other studies, maternal mortality has been as high as 17%, and fetal mortality has been as high as 59%. The incidence of uterine rupture has been noted as follows: Margulies and Crapanzano 2 report an incidence of 1:3232; Weingold, Sanford and Sherman 3 report 1:1310; Yussman and Haynes4 report 1:1230; Beachman, Beachman and Webster" report 1:12240; and Schrinsky and Benson 6 report 1:2695.
